Complete the form below.  You will still be required to complete a printed form listed below as well that you can download now and fill out or fill out at the studio the first day of camp or on the day of the workshop.  The same forms can be used for multiple children of the same caregiver.  Payment will confirm your childs registration and can be paid by phone using a visa or mastercard or you can pay in person at the studio.
